Keyworker

As a resident at Waltham House you will have your own dedicated keyworker through our Keyworker service.

What is a Keyworker Service?

Shortly after your arrival you will be introduced to your keyworker whose job it is to get to know and support you, and if applicable, your family. They’ll help you settle in, make changes to your room to make it a home-from-home, show you round and introduce you to your fellow residents.

They will be committed to ensuring that your needs, preferences and lifestyle choices, as well as expectations are met. They will act as your point of reference for care and consult with you and your family/carer regularly to ensure you receive the best possible standard of care. They will do this through the provision of a care plan.

What is a Care Plan?

A care plan is a comprehensive document that we use get to know you. Along with the manager, your keyworker will work through it with you to understand your needs from personal hygiene to medication, any routines, likes and dislikes and what’s important to you. We will use your care plan as a reference guide to ensure you receive the same level of care and attention from all our staff and visiting staff you may come across during you time at Waltham House Care Home, Grimsby.

Your keyworker will periodically review your care plan with you to help you make any changes if applicable, and support you to achieve your goals. This is something that can be done as often as is necessary and should you require access to your care plan it’s available at any time.
